Talkspace scores 7.5/10. It connects you with therapists licensed in your state, lists 5,600+ licensed providers, adds psychiatry and medication management, and is in-network with major insurers including Medicare plans. Out of pocket, therapy plans start at $69 a week. Its evidence is the strongest of the therapy platforms we reviewed: in a randomized trial of 83 adults with depression, message-based therapy was noninferior to weekly video therapy, and a larger 215-person trial on Talkspace found similar results with no adverse events. Talkspace employees co-authored some of this work, and internet-delivered CBT in general has been shown to match face-to-face CBT across 20 trials. It loses points on transparency because the homepage says most insured members pay $0 while also saying copays are typically $15. Talkspace itself says not to use the site in a life-threatening situation: call or text 988 in the US.

What the evidence says
- In a randomized trial of 83 adults with depression, message-based psychotherapy produced medium-to-large improvements in depression and was noninferior to once-weekly video psychotherapy. Two authors were Talkspace employees. (Song et al., J Med Internet Res 2023 (PubMed))
- In a randomized trial of 215 adults with depression treated on Talkspace, message-based and video-based psychotherapy showed no differences in rates of change in depression or anxiety by 6 or 12 weeks, and neither caused adverse events. (Arean et al., Psychiatric Services 2024 (PubMed))
- A meta-analysis of 20 trials (1,418 participants) that directly compared guided internet-delivered CBT with face-to-face CBT found equivalent overall effects (Hedges g = 0.05), though the authors say few conditions have been studied individually. (Carlbring et al., Cogn Behav Ther 2018 (PubMed))
