Our verdict

Apple AirPods Pro 3 with the Hearing Aid feature leads our 2026 OTC hearing aid ranking at 7.8/10, and Firsthand earns nothing on it. The FDA granted the Hearing Aid feature De Novo authorization in September 2024 for adults 18 and older with perceived mild to moderate hearing loss, after a study of 118 people found self-fitting gave similar perceived benefit to a professional fitting of the same device, with no adverse events reported. A separate 2026 study found Apple's Hearing Test came within 10 dB of booth audiometry on 86.5% of thresholds. At $249 it is the lowest-cost FDA-authorized option here. The limits matter: it needs a compatible iPhone or iPad, amplification only runs in Transparency mode with no media playing, battery life is up to 10 hours, and Apple's return window is 14 days with no audiologist follow-up. If you hear well in quiet but struggle in groups and you already use an iPhone, it is a sensible first step. Sudden, one-sided or painful hearing changes need a clinician, not an earbud.

What the evidence says

  • FDA authorized the Hearing Aid Feature on September 12, 2024 for adults 18+ with perceived mild to moderate hearing loss. In a 118-person study, self-fitting users had similar perceived benefit to those who were professionally fitted, comparable in-ear amplification and speech-in-noise results, and no adverse events. (FDA press release: FDA Authorizes First Over-the-Counter Hearing Aid Software)
  • FDA's De Novo database lists DEN230081, Hearing Aid Feature (HAF), requester Apple Inc., classified as air-conduction hearing aid software, decision granted September 12, 2024. (FDA De Novo database: DEN230081)
  • In 25 adults with self-reported mild to moderate hearing loss, 86.5% of Apple Hearing Test thresholds on AirPods Pro 2 fell within 10 dB HL of booth audiometry, test-retest was reliable, and the test took a median 5.5 minutes versus 10 minutes. Pros & cons

Pros

  • The Hearing Aid feature was granted FDA De Novo authorization (DEN230081) in September 2024 as the first OTC hearing aid software
  • In the FDA-reviewed study of 118 adults, self-fitting gave similar perceived benefit to professional fitting of the same device, with no adverse events reported
  • At $249 it costs a fraction of dedicated OTC hearing aids, and it is also a pair of everyday earbuds
  • Built-in Hearing Test, or you can load an audiogram from your own hearing professional

Cons & red flags

  • Needs AirPods Pro 2 or 3 plus a compatible iPhone or iPad; there is no Android path
  • Apple says amplification only works in Transparency mode and when no media is playing
  • Up to 10 hours per charge with the Hearing Aid feature, which is short of an all-day wear for some people
  • Apple's standard return window is 14 days, much shorter than the 60 to 100 day trials offered by hearing aid makers, and there is no audiologist support

Frequently asked questions

Are AirPods Pro really an FDA-authorized hearing aid?

The Hearing Aid feature software is. FDA granted it De Novo authorization (DEN230081) on September 12, 2024, as over-the-counter hearing aid software for adults 18 and older with perceived mild to moderate hearing loss. It runs on AirPods Pro 2 and AirPods Pro 3.

Can I use it while listening to music?

No. Apple's support page says amplification only works in Transparency mode and when no media is playing on the paired device.

Do I need a hearing test first?

You can take Apple's built-in Hearing Test or enter audiogram results from your own hearing professional. A 2026 study found the built-in test was within 10 dB of standard audiometry for 86.5% of thresholds in 25 adults.
